ICC Participating OrganizationsAUSTIN/TRAVIS COUNTY HEALTH AND HUMAN SERVICES DEPARTMENTThe AT/CHHSD's purpose is to work in partnership with the community to promote health, safety, and well being. They provide Public Health, Social, and Environmental Health services for Austin and Travis County, and serve a population of nearly one million.AUSTIN TRAVIS COUNTY MENTAL HEALTH MENTAL RETARDATION CENTER (ATCMHMR)The Center provides community-based mental health, mental retardation and substance abuse services to adults and children who are the most severely disabled and who are most in need of services. A comprehensive array of services is delivered through four networks and three support divisions located in over 60 facilities and numerous sub-contracted entities across the city and county. These services include information and referral, psychiatric evaluation, 24-hour crisis intervention, medication support, inpatient treatment, employment and vocational services, care coordination, service coordination, family support and respite care, housing, supported living and residential services. Additionally, Center staff often provide mobile services in consumers� homes, on the streets, or at other community sites. The Single Point of Entry (SPOE) has been developed as a uniform telecommunications system linking all the divisions of the Center.AUSTIN WOMEN'S HOSPITALIn February 2002, the Austin City Council approved the creation of an acute care facility on the fifth floor of Brackenridge Hospital at 601 East 15th Street. The Austin Women's Hospital opened in January, 2004, and is operated by the University of Texas Medical Branch (UTMB). Entrance to the facility is via elevators located on Hospital Drive. For more information, call 512/322-2100.CENTRAL TEXAS MEDICAL CENTERCentral Texas Medical Center is located in Hays County and serves approximately 2,210 medically indigent yearly. There are no private health clinics providing care for the medically indigent in Hays County. The Hill Country MHMR Center assists individuals with severe mental health problems.CITY OF AUSTIN COMMUNITY CARE SERVICES DEPARTMENTThe Community Care Services Department operates the Austin/Travis County Federally Qualified Health Centers to provide customer-oriented, quality-driven, and cost-efficient healthcare services for underserved individuals to meet their health needs.EL BUEN SAMARITANOSince 1987, El Buen Samaritano Episcopal Mission has fulfilled a unique role in Austin as a provider of integrated health care, emergency food, advocacy, leadership, development, and basic education for working-poor Hispanic families. At El Buen Samaritano, the mission is to transform lives by engaging the strengths of working-poor Hispanic families to promote their successful participation in society.JOHNS COMMUNITY HOSPITALJohns Community Hospital is a 58-bed general acute care hospital serving Taylor and the surrounding rural area of East Williamson County. The hospital has a 24-hour physician staffed Emergency Room. Attached to the hospital is the Health Center, a multi-specialty clinic for patients of all ages, which has six physicians, two family nurse practitioners and a number of visiting specialists.LONE STAR CIRCLE OF CARELone Star Circle of Care began as an idea for a locally-focused community clinic, a place that would treat the large number of medically underserved individuals in Georgetown, Texas. When Georgetown Community Clinic opened in January 2002, the dedicated group of concerned citizens who saw their idea through did not know what great challenges awaited them. For the first time, uninsured residents had a medical home where they could receive affordable, quality health care. People not only from Georgetown, but from all ver Williamson County, sought medical care at our clinic. We are charged with becoming a county-wide safety net of care for our patients. In anticipation of continued growth, our organization has adopted the parent name Lone Star Circle of Care. All clinics will receive FQHC benefits under the LSCC umbrella.PEOPLE'S COMMUNITY CLINICPeople's Community Clinic is a private non-profit primary care clinic that provides medical treatment and prevention services for low-income uninsured and underserved adults and children. They have two outreach clinic sites target high-risk youth. People's serves as medical home for approximately 11,000 patients.PLANNED PARENTHOOD OF THE TEXAS CAPITAL REGIONPlanned Parenthood of the Texas Capital Region, Inc. is a non-profit health care organization providing high quality sexual health care and family planning services, at affordable prices, to men and women of all ages; resources and programs for sexuality education; advocacy to ensure reproductive rights, access to family planning, and comprehensive sexuality education.ROUND ROCK HEALTH CLINICRound Rock Health Clinic provides primary health care services to medically underserved children, women, and adults. The clinic strives to build the foundation for a healthy and productive community by ensuring that pregnant women have access to high quality prenatal care and that children receive comprehensive health screenings and preventative care.ST. DAVID'S HEALTH CARESt. David's HealthCare is a partnership between the not-for-profit St. David's HealthCare System and HCA - The Healthcare Company, the nation's largest provider of health care. St. David's HealthCare provides quality, compassionate care through four comprehensive acute-care hospitals in Austin--St. David's Medical Center, North Austin Medical Center, South Austin Hospital, and Round Rock Medical Center as well as outpatient surgery centers, a rehabilitation center, a psychiatric facility and occupational health services.SETON FAMILY OF HOSPITALSThe Seton Family of Hospitals, a not-for-profit organization, is the leading provider of healthcare services in Central Texas, serving an 11-county population of 1.4 million. The network includes: five urban acute care hospitals, including the only Level II Trauma Center in Central Texas; two rural hospitals; a mental health hospital; several strategically located health facilities that provide medical care for well patients; three primary care clinics for the uninsured.TRAVIS COUNTY HEALTHCARE DISTRICTThe Travis County Healthcare District (TCHD) is a limited-purpose taxing district responsible for providing healthcare to indigent persons residing in Travis County, Texas. It is the mission of the Travis County Healthcare District to promote the health and wellness of the residents of the community, especially the uninsured and underinsured, by working together to ensure access to a full range of coordinated healthcare services. The Travis County Healthcare District was created by a vote of Travis County residents on May 15, 2004.TRAVIS COUNTY MEDICAL SOCIETYThe Travis County Medical Society (TCMS) is a component society of the Texas Medical Association (TMA), a statewide professional organization of licensed physicians. With more than 2,500 members, the TCMS represents approximately 90% of all practicing physicians in the Greater Austin area. The TCMS represents the medical profession in matters of importance to the membership and to the community it serves Project Access The Travis County Medical Society, working with the Indigent Care Collaboration, has initiated PROJECT ACCESS, a coordinated system of volunteer physician care, hospital care, diagnostic services and medications assistance for the low-income, uninsured of Travis County. As of August 2006, 870 participating physician volunteers of Project Access continue to provide access to health care for low-income uninsured patients along with program partners such as the Seton Family of Hospitals, St. David's Healthcare, the Heart Hospital of Austin, Austin Radiological Association, Clinical Pathology Labs, Inc., and Austin Health Imaging. Since commencing operations in early 2003, Project Access has enrolled 1375 patients, provided over 6,600 patient encounters, resulting in over $5.2 million in donated care. Project Access patients have also received over $360,000 in free medications through patient prescription assistance programs. UT SCHOOL OF NURSINGThe University of Texas at Austin School of Nursing is heavily involved in community activies: both on campus and in and around Austin. This section highlights the School of Nursing's relationship with the community health clinics, the city of Austin and the ongoing attempts of School of Nursing faculty and staff to recruit local people into the profession of nursing.VOLUNTEER HEALTHCARE CLINICThe Volunteer Healthcare Clinic serves as a walk-in primary care clinic on Tuesday and Thursday evenings and an appointment-only chronic disease management clinic on Monday evenings. The Clinic provides care to the working poor who meet the income guidelines.
